Our digital patient onboarding formĀ ensures patient information, including contact details and medical history, is stored securely and privately.Ā You can utilise this form in advance of appointments to ensure workers get the most out of their time in the treatment room.
Checking that we have the correct paperwork at the beginning of an appointment can sometimes impinge on how long we can spend treating a patient. To minimise this impact, and to complement our digital form, we have recently introduced an on-screen consent form with signature functionality.
When Work Healthy Australia asks for consent from a patient, this isn’t just for treatment. Their consent also allows us to collect and store their information (in accordance with privacy laws), and it also gives us their permission to share and report back to their employer on any relevant health conditions that may impact their ability to perform their role.
When a patient arrives at their appointment, the consent form will be displayed on the Work Healthy Australia provider’s computer screen for them to complete and sign. A digital copy of the patient’s consent form is then generated and stored against the patient’s record in our system. The patient can request a copy of their consent form for their own records, and it will be easily accessible in future should the need arise.
